Please bring your picture ID and insurance card to all appointments. Co-payment is due at the time of service. Patients under the age of 18 must be accompanied by a parent or legal guardian at their first appointment. If they are not, the appointment will be rescheduled. Legal guardian or POA must accompany the patient at each visit

We have transitioned to electronic medical records, please use the Patient Portal button to see your visit, update your medical history and view your medical records online.
Please use the safe link on our website for online payment. DOXO bill payment warning: There is an online bill pay website that is called Doxo.com. It is a third-party website that allows people to pay their bills via their website. DOXO is NOT affiliated in any with Dermatology Associates of West Michigan. DOXO may charge additional fees, and we cannot control when or if payments will be received.
Telehealth Appointments
Starting April 1st, 2025, we will no longer offer telehealth consultations. Please contact us at 616-949-5600 or visit our online booking system to schedule an in-person appointment.
If we are not contracted with your insurance plan, payment is expected in full at the time of service. We accept cash, check, MasterCard, Visa, Discover, and American Express.
We are contracted with the following insurance carriers: Aetna, Blue Cross Blue Shield Plans, Blue Care Network, Medicare, PhysiciansCare Health Plans, Cofinity (formerly PPOM), Priority Health (excluding Medicaid and Michild), Aetna Medicare, Tricare/CHAMPVA, BCN Advantage, Medicare Plus Blue PPO, PriorityMedicare, Railroad Retirement Board, Federal Blue Cross Blue Shield, United Health Care, CIGNA, VA-VACCN, Health Alliance Plan (PPO/EPO only).
We do not accept any form of Medicaid.

Cancellation Policy
If you are unable to make your appointment, we do require 24-hour notice of any change or cancellation of your appointment.
